• Release date (date when Approved Supplier List was first published)
• Revision no (the number of times the Approved Supplier List has been revised)
• Revision date (the date the Approved Supplier List was last revised)
• Page number (Number of pages of Approved Supplier List)
• Name and address of the supplier
• Purchased products and services
• Date of registration (starting date of working with the supplier)
• Release date (date of removal from supplier)
• Reason for exit (Reason for removal of supplier)
• Prepared (Name, surname and title of the approved Supplier List)
• Checker (Name, surname and title of the person checking the Approved Supplier List)
• Approved (Name, surname and title of the person who approved the List of Approved Suppliers)
